Payment Policies
- A 10% deposit is required at the time the evaluation appointment is scheduled.
- Payment is due at the time services are rendered. Results will not be released until full payment is received.

Insurance Information
Dr. Lake is not paneled with insurance providers. This is intentional, as many insurance companies restrict the type and scope of evaluations that can be performed.Â
To ensure the highest quality and most comprehensive assessment, evaluations are conducted independently.
Families are encouraged to contact their insurance company in advance if they wish to seek reimbursement. Following completion of testing, Dr. Lake can provide appropriate billing and diagnostic codes when applicable.
